5/30/2023 EnchantaVaughn Approved (AM) Upon arrival, client was observed settling into class, and preparing for morning work. Client demonstrated excellent participation and was able to complete his work on his own doing a much better job with working independently. Client appeared to be having a good start to his day, as evidenced by him attending to all instructions and being respectful to authority figures without getting upset or frustrated. Client displays a proactive attitude when talking about activities he's interested in. Client discussed positive thoughts about being in school today. Client transitions to lunch displaying a pleasant attitude and exhibiting good manners. (PM) Client was monitored and observed interacting well with peers and staff. Client expressed a positive attitude while engaging in conversation with his male friends. Client was seen cleaning trash off table and waiting for directions. Teacher communicated that client is allowed to attend group session everyday after lunch to accommodate his needs in order to meet his behavior expectations. Client was able to participate in recess and selected games, displaying excellent social skills. Client was supported for keeping his hands to himself and being a team player. Client shifted back into the classroom setting to begin pm studies. Client was seen modeling age appropriate behaviors with his group during assigned stations. (EOD) Client appears to grasp a clear understanding of expectations and acknowledges his personal strengths and weaknesses through role playing exercises and psychoeducational activities. Client displays appropriate social skills in class. Client appeared to be in a delighted mood, as evidenced by his ability to stay focused and complete his work on time. Client displayed appropriate classroom mannerisms (i.e. raising his hand, paying attention, and being a team player) and was supported for his cooperation. Client demonstrated that he was having a wonderful time in school today. Client organized and cleaned off his desk while listening to evening announcements. Client was observed paying attention to his teacher and being cooperative while waiting for walkers to be dismissed. Client behaviors were acceptable at this time requiring little to no direction. Client waved goodbye to his counselor and was observed exiting the building in a safe manner; meeting objectives 1, 2, and 3.
